Please Note: No computer / iPad is to be removed from a room in the New Lexington City School District without email approval from the District Technology Director upon receipt of this form. When a computer is checked out for summer time home use, the borrowing staff member is fully responsible for the computer / iPad's safe return and installation, one week prior to the start of the school year.
